Health Clearance Requirements

New Undergraduate Students

Log in @ https://cui.medicatconnect.com and complete the following requirements. Failure to comply by the deadline will result in a Health Hold that may prevent you from enrollment and moving to housing.

  1. Complete Health History Form
  2. Enter dates for the following requirements:
    1. MMR #1 (Measles, Mumps & Rubella) vaccine or positive titers
      • received after 1 year of age
    2. MMR #2 (Measles, Mumps & Rubella) vaccine or positive titers
      • at least 4 weeks after the first dose
    3. Tdap (Tetanus, diphtheria & pertussis) vaccine
      • received in 2005 or later
    4. Meningococcal conjugate (A/C/Y/W-135) vaccine
      • administered at age 16 or older
    5. TB (tuberculosis) clearance
      • completed within the last 12 months
      • acceptable forms are TB skin test, lab test, or chest x-ray
  3. Submit proof by uploading a picture of immunization record that contains the dates entered above

All of these are done online @ https://cui.medicatconnect.com. We do not accept fax or e-mail copies.

Recommended Immunizations

  • Meningococcal serogroup B
  • Hepatitis A
  • Hepatitis B
  • Varicella (chickenpox)
  • HPV (human papilloma virus)

Deadline for completion

  • Fall semester: August 1
    • In light of the challenges brought on by COVID-19, the deadline for submission of immunization records for Fall 2020 has been extended to October 1, 2020. Health History Form still needs to be completed by August 1, 2020. Log in to cui.medicatconnect.com to fill out your health form online and start uploading available immunization records.
  • Spring semester: December 1

A fee of $50 will be assessed each time that a temporary 24-hour health clearance is requested (moving to housing, adding or dropping a class, making a schedule change, etc...). No request will be processed beginning November 1 for Fall semester and March 1 for Spring semester.
