Health Clearance Requirements

New Undergraduate Students

Log in @ https://cui.medicatconnect.com and complete the following requirements. Failure to comply by the deadline will result in a Health Hold that may prevent you from enrollment and moving to housing.

1. Complete Health History Form
2. Enter dates for the following requirements:
   1. MMR #1 (Measles, Mumps & Rubella) vaccine or positive titers
      • received after 1 year of age
   2. MMR #2 (Measles, Mumps & Rubella) vaccine or positive titers
      • at least 4 weeks after the first dose
3. Submit proof by uploading a picture of immunization record that contains the dates entered above

All of these are done online @ https://cui.medicatconnect.com. We do not accept fax or e-mail copies.

Recommended Immunizations

• Meningococcal serogroup A/C/Y/W and B
• Hepatitis A
• Hepatitis B
• Varicella (chickenpox)
• HPV (human papilloma virus)
• Tdap

Deadline for completion

• Fall semester: August 1
• Spring semester: December 1
