Required Supporting Documentation

In addition to a completed application, the following items will be needed to complete your admission file:

Preliminary Single Subject / Multiple Subject and Education Specialist/Dual Credential

  1. Completed Application: Apply Online
  2. $50 non-refundable application fee
  3. Official transcripts (e-transcripts preferred) from each college/university attended
  4. Statement of Intent
  5. Two References: Send references with Online Form (Candidates must use this form)
  6. Certificate of Clearance & Livescan fingerprinting using this this form for Teacher Credential Program
  7. Negative tuberculosis (TB) test (within 6 months of the first night of class)
  8. Basic Skills Requirement met
  9. Submission of the Basic Skills/Subject Matter Pre-Evaluation Form with supporting documents

Clear Induction Credential Program

Application deadline is August 1st, but the start of the program is not until October 17th. This allows students to settle into the new semester before starting the program.

  1. Completed Application: Apply Online
  2. $50 non-refundable application fee
  3. Two References: Send references this Online Form (Candidates must use this form)
  4. Copy of valid CA Preliminary Credential
  5. Proof of full time employment as Teacher of Record for the upcoming school year (Official letter on letterhead by School Administrator/HR verifying years of employment, teaching assignment, and full time or part time placement. )

Students with International Transcripts

ALL non-US equivalent transcripts must arrive with a certified English evaluation and translation, and evidence of degree posted. The student must have a minimum GPA of 2.75 on a 4.0 scale, or equivalent to that according to the grading system used by the country that a student is applying from.

Please note that CUI does not have a translation or evaluation service, and students are responsible to get an official translation and evaluation done by a NACES-approved evaluation agency verifying the equivalent of a regionally accredited U.S. bachelor's degree or higher – CUI prefers international transcripts to be evaluated through IERF.
