Consent to be photographed – videotaped

I am the parent or legal guardian of the Participant. I understand the legal consequences of signing this document, including the consent to media coverage and publication without compensation. I allow Participant to participate in the activity. I understand that I am responsible for the obligations and acts of Participant. I agree to be bound by the terms of this document.

I hereby submit my consent for Concordia University Irvine to take photographs and videos of the Participant and to distribute and/or exhibit all or any part thereof without restriction, in perpetuity. On behalf of the Participant and myself, I consent to the use and publication of such photographs, pictures, slides, movies, videos, or other media coverage for any legal purpose by the University, without compensation from the University. In addition to using such photos and videos for educational purposes, I submit my consent for Concordia University Irvine to use such photos and videos in promotional materials and advertising (including, but not limited to, print, online, mobile, outdoor, direct mail and email ads), on University social media applications, and on the University’s website.

I agree to indemnify and hold harmless Concordia University Irvine their employees and representatives against any and all claims arising out of such presentation, including, but not limited to, claims of copyright infringement, invasion of privacy, defamation, and misrepresentation.

All photographs and videos will be the property of Concordia University Irvine.
