Adobe Fillable Form Signing Instructions

1. Download the form and open it from your downloads folder.
2. Sign the form electronically.
   • If you have used the signature feature before, please enter your PIN for signature, date the form, and skip to #9 in the procedures below.
   • If this is your first time to sign a fillable form, you will receive this popup when you click on Instructor Signature because you need to set up your signature.
3. Click Configure Digital ID
   
   
4. Click Create a new Digital ID
   
   
5. Click Save to File
   
   
6. Complete the information on the next screen
   
   

   Choose Digital Signatures in the last field.

7. Choose a file location and password.
   
   
8. Click Continue
   
   
9. Enter your PIN, sign and date the document, and save it.
10. Email the signed document to the person designated on the form. You will receive an email notification once the form has been processed.
