CSA Report Form

The Clery Act requires all Campus Security Authorities (CSA) to immediately report crimes to the Department of Campus Safety as soon as they are made aware of them. The contents of this form will be sent to the Director and Associate Director of Campus Safety for review. For accuracy of reporting Clery crime statistics, please fill out this form completely. This form should not be used to report crimes in progress or incidents that pose an immediate threat to the larger community. Reports submitted through this web form may not receive an immediate response. If a crime requires an immediate response, please call 9-1-1 and then Campus Safety Dispatch at 949-214-3000. For other immediate assistance, please contact Campus Safety Dispatch. This form is only for faculty, staff, and student employees who are considered Campus Security Authorities.
