Services Offered
- Assessment and Treatment of Injuries and Illnesses
Guided by policies and procedures developed in collaboration with a consulting physician and university administration, the Nurse Practitioner (NP) provides assessment, diagnosis, and treatment for common illnesses, injuries, and certain psychiatric illnesses. When a student’s needs extend beyond the scope of services offered, referrals are provided to appropriate community healthcare providers covered by their insurance whenever possible. - Point of care testing such as flu, mono, and strep
- TB (Tuberculosis) Testing: TB tests (PPD-Mantoux) are given on Monday, Tuesday and Wednesday and read two days later.
- Nurse Practitioner consultation on health topics
- Labs: Nurse Practitioners can provide laboratory orders for STI/STD screening, sickle cell screening, QuantiFERON-TB Gold testing, and vaccine titers. Students requiring these tests are referred to Quest Diagnostics, where specimens are collected at off-campus laboratory locations. Quest Diagnostics bills most insurance providers directly; however, students may be responsible for costs not covered by their insurance. Students are encouraged to verify coverage with their insurance provider and confirm whether Quest Diagnostics is an in-network laboratory before testing.
Things to Know
- All visits and records are confidential.
- Medications: SHS provides free samples of select over-the-counter medications for common illnesses and injuries, including decongestants, cough medications, throat lozenges, acetaminophen, and ibuprofen. Antibiotics may be purchased
- for approximately $20. When needed, prescriptions can be electronically sent to the student’s preferred pharmacy, with the exception of Kaiser pharmacy.
- Office visit charges: There is no charge for an office visit for full-time undergraduate students as this is part of their University fees. There is a fee of $25.00 for medical office visits for part-time and graduate students which must be paid at time of service.
- TB (Tuberculosis) Testing (make this bold): There is a $40 fee for TB testing. There is no charge for the reading.
Health Clearance Requirements
New Undergraduate Students
Log in @ https://cui.medicatconnect.com and complete the following requirements. Failure to comply by the deadline will result in a Health Hold that may prevent you from enrollment and moving to housing.
- Complete Health History Form
- Enter dates for the following requirements:
- MMR #1 (Measles, Mumps & Rubella) vaccine or positive titers
- received after 1 year of age
- MMR #2 (Measles, Mumps & Rubella) vaccine or positive titers
- at least 4 weeks after the first dose
- MMR #1 (Measles, Mumps & Rubella) vaccine or positive titers
- Upload a copy of your immunization record that verifies the dates entered above. The student’s name must be clearly visible on each uploaded document. Records submitted without a name will not be accepted.
All of these are done online @ https://cui.medicatconnect.com. We do not accept fax or e-mail copies.

- Meningococcal serogroup A/C/Y/W and B
- Hepatitis A
- Hepatitis B
- Varicella (chickenpox)
- HPV (human papilloma virus)
- Tdap

- Fall semester: August 1

Patient Rights and Responsibilities
All of our patients and clients have the right to privacy and confidentiality. As a consumer, you have the responsibility to be honest and direct in a courteous fashion about everything that relates to your needs for health services. Exceptions are determined by California and Federal law, which directs us to report if you or someone else is in imminent danger or if a minor or impaired person is being abused.
Patient Rights
- You will be treated with respect, dignity, and without discrimination.
- You have the right to privacy—in examinations, records, and communication.
- You will receive clear and complete information to make informed decisions, including refusing treatment.
- You have the right to participate in care planning.
- You may file a complaint confidentially, without affecting your care.
Patient Responsibilities
- Provide accurate health and insurance information.
- Respect staff, other patients, and facility property.
- Follow the treatment plan or inform your provider if unable.
- Understand that charges must be paid at the time of service and discuss financial concerns upfront.
- Keep or cancel appointments with at least 24‑hour notice.
- Follow facility rules (e.g., visitor, smoking), and handle billing matters promptly.

Being a good health care consumer does not mean being a silent partner.
