Required Supporting Documentation

Reference Letter

• Reference (PDF) Must be completed by applicant’s counselor, teacher or employer.
  

Official High School Transcript

• Official transcripts must be sent directly from the high school or college to Concordia University in a sealed envelope.
  

Official Scholastic Aptitude Test (SAT) or the American College Test (ACT) scores (Freshmen Only)

•  The scores should be sent to the Admission Office. The university’s code numbers for the tests are: SAT 4069; ACT 0227.
