After reviewing the information on the Family Educational Rights and Privacy Act of 1974, please...
- Print the acknowledgment form only if your need to have an Internet Native Banner Account (check with your supervisor first).
- Complete and sign the form indicating that you understand the information contained in this training session.
- Email or mail the form to the Registrar's Office as indicated.
- When the form is received by the Registrar's Office, we will contact IT so that access to Banner can be granted.
