Student Health Insurance Program for 2012-2013
Concordia University Irvine (CUI) has a mandatory student health insurance policy for all of its full-time undergraduate students, athletes, nursing students, and international (graduate & undergraduate) students. All of these students are enrolled in the Concordia University Irvine-Sponsored Student Health Insurance Plan (SHIP) for the purpose of limiting undue financial and academic stress related to illnesses during the academic year. It is our belief that an adequate health policy will assist in meeting not only common illnesses treated at the student Wellness Center or at a private physician's office, but also with the financial burden of illness and accidents involving more extensive care. However, if you have adequate health insurance through your work or parents that meet the waiver criteria enumerated below, you have the option to waive out of the University-Sponsored SHIP.
While all traditional undergraduate students on the main campus are automatically enrolled in the University-Sponsored SHIP, non-traditional students (CU Accelerate) and graduate students (except for international students) are not automatically enrolled; however, they are encouraged to carry health insurance. As an option, non-traditional students are eligible to enroll in the University-Sponsored SHIP.
Concordia University Irvine students who wish to carry their private health insurance must submit an electronic waiver annually; otherwise they will automatically be enrolled in the Concordia University Irvine SHIP and the insurance premium will be billed to their student account.

Fall 2012 Students
The Concordia University Irvine-Sponsored SHIP costs $1,650 for annual coverage from August 1, 2012 to July 31, 2013. This amount will automatically be billed to the student's account.
Spring 2013 Students
The Concordia University, Irvine-Sponsored SHIP costs $957 for spring/summer coverage from January 1, 2013 to July 31, 2013. This amount will automatically be billed to the student's account. For new freshmen and transfers in the 2013 Spring Semester, you will have to waive out or enroll once again in the 2013 Fall Semester.
Waiving the Insurance Plan
The following criteria must be met in order to waive the student health insurance plan:
- The plan is owned, headquartered and operated in the US.
- The Plan provides primary care services within 50 miles of Concordia University, Irvine. Coverage for emergency-only care does not satisfy this requirement. You will be asked to provide your local healthcare provider's name, clinic address, and phone number.
- The plan pays for hospitalization in the US.
- The plan covers inpatient and outpatient mental health in the US.
- The plan has an annual individual deductible of $1000 or less.
- The maximum out-of-pocket expense is $5,000 per individual per year.
- The maximum annual benefit is at least $100,000.
- The coverage will remain in effect throughout the 2012-2013 academic year.
If a student has their own coverage, an online Health Insurance Waiver
needs to be completed anytime between December 17, 2012 and January 31, 2013
to waive the Student Health Insurance Plan.
Health Insurance Waiver will not be accepted after the January 31, 2013
deadline. Students who do not request to have the insurance waived will
automatically be enrolled in the Concordia University Irvine Student
Health Insurance Plan.
* You have to be enrolled in at least 12 units before the waiver
site will allow you to log in.
To enroll in the insurance plan:
No further action is necessary. All full time undergraduate, nursing, and international students are automatically enrolled unless a waiver is submitted online.

General Information
Health insurance is not needed for care received from the Health Center. We do not bill insurance companies.
We recommend that all students understand their insurance availability within Orange County. There are many types of insurance policies and it is the student's responsibility to understand their coverage. The student handles all health insurance matters directly with their insurance company for off-campus medical care.
Each student is legally responsible for payment of all fees for medical and mental health expenses incurred for the academic year at Concordia University Irvine.
Concordia University, Irvine assumes no liability for student illness or injury and no responsibility for medical services contracted for by individual students.
