Health Form (New Students)

All new undergraduate CUI students are required to complete and submit Health Forms.  Please send your completed Undergraduate Health Form to the Wellness Center to ensure that a HOLD will not be placed on your account. A HOLD on your account will hinder your ability to complete the registration process and prevent you from moving into housing. Please schedule your doctor appointments for needed physicals and immunizations immediately to complete your forms.

The Wellness Center is open during school sessions and closed on student breaks and holidays. Should you have any questions regarding these forms, please contact (949) 214-3102 or wellness.center@cui.edu, Director of Health Services.

Student Health Insurance

There are no more forms to fill out! To enroll or waive, please go to www.cui.edu/insurance and follow the prompts. The online window is open on the first Monday in July – August 31 for the fall semester and on the first Monday in December – January 31 for the spring semester. All full time undergraduate students, including athletes and nursing students, are automatically charged a student health insurance fee unless it is waived online during the open window.

Post Hospitalization Clearance Form

Any student who gets hospitalized or taken to the Emergency Department needs to fill out a Clearance Form before moving back to housing and/or coming back to class.

Psychological Counseling Forms

Students who wish to see a counselor will need to fill out an Intake Packet. This needs to be handed to the therapist on the first counseling session. Hard copies are also available at the Wellness Center.

Sessions are kept confidential. However, there are cases wherein the student requests that the Wellness Center therapist speak to his/her private therapist, parents, professor, etc… In this case, a Consent to Release Information will need to be filled out by the student.
